3. P/U Time

4. P/U Location. (ex: residence, 123 Main st., Hillsboro, etc.)

 If A PDX P/U location, then you must include all of the following:

1. landing time (do not include pickup time above)

2. airline

3. flight #

4. departure city

5. D/O Location (ex: residence, 123 Main st., Hillsboro, etc.)
